</span><span>manufacturer</span><span> </span><span>provided</span><span> materials and</span><span> product</span><span> monograph</span><span>;</span></span><span> </span></p></li><li><p><span><span>Work with patients and families to complete a needs </span><span>assesment</span><span> related to medication access;</span></span><span> or product related questions, based on information provided as part of the program materials to ensure patient adherence, as dictated by the needs of the </span><span>patient</span><span>. </span></span><span> </span></p></li><li><p><span><span>Refer the patient to contact their healthcare provider should the patient have clinical questions, assessments and situations that fall outside program </span><span>provided</span><span> materials or manufacturer product monograph; </span></span><span> </span></p></li><li><p><span><span>Database management and ongoing auditing of files and information </span><span>captured;</span></span><span> </span></p></li><li><p><span><span>Administer</span><span> </span><span>and</span><span> </span><span>adhere</span><span> </span><span>to</span><span> </span><span>case-management</span><span> </span><span>protocols,</span><span> </span><span>including</span><span> </span><span>data</span><span> </span><span>collection,</span><span> </span><span>on-going patient follow-up, </span><span>therapy disruption/discontinuation monitoring, adverse event reporting and liaison with primary healthcare </span><span>agencies</span><span> </span><span>as</span><span> </span><span>required,</span><span> </span><span>to coordinate infusions, drug delivery and resolve issues as they databases, processes, inter-departmental </span><span>relationships</span></span><span> </span></p></li><li><p><span><span>Report Adverse Events / Severe Adverse Events following program Standard
