Job Title: Psychosocial Rehabilitation (PSR) Provider

Departments: CFTSS

Purpose of the Position:

The PSR role provides rehabilitative services that aim to enhance the social, emotional, and functional skills of

children/youth, fostering independence and promoting integration within family and community settings.

Reports To: Assigned CFTSS Supervisor

Supervises: Not Applicable

1. Specific Responsibilities

  • Engage children/youth in activities that develop social, emotional, and daily living skills to improve functioning

within the community.

  • Implement structured interventions that support independence, enhance interpersonal skills, and promote

positive behaviors.

  • Educate and support families in reinforcing these skills at home and in the community.
  • Facilitate the development and monitoring of individualized service plans with clear, achievable goals.
  • Attend monthly waiver service provider meetings to ensure consistency in service delivery and stay informed on

updated practices and protocols.

  • Document services provided, including progress notes, service plans, expense logs, and other required records,

submitted within prescribed timeframes.

  • Report any incidents or concerns immediately to the supervisor to maintain client safety and service

transparency.

  • Coordinate with other service providers and the Care Manager to discuss progress, address challenges, and

ensure continuity of care.

  • Assess the suitability of community or home settings for providing services, tailoring interventions to meet the

child/youth’s needs.

2. Qualifications

  • At a minimum, staff must be 18 years old and have a high school diploma, high school equivalency preferred, or

a State Education Commencement Credential with a minimum of two years’ experience in children’s mental

health, addiction, foster care and/or a related human services field; or

  • A bachelor’s degree in social work, psychology, or in related human services; or
  • A master’s degree in social work, psychology, or in related human services
  • Experience in rehabilitative or human services, particularly with children/youth.

3. Preferred Qualities

  • Must be fluent in Yiddish and English
  • Culturally sensitive and collaborative within team settings.
  • Professional approach, maintaining boundaries
  • Open to receiving and applying constructive feedback.
  • Strength-based, client-centered approach.

Pay: $30.00 - $35.00 per hour
