Interim HealthCare is seeking a compassionate 1:1 Overnight LPN to provide dedicated home-based nursing care to a single patient during overnight shifts. As part of our Healthcare & Medical Services team, you will deliver attentive, patient-centered care in the comfort of the patient’s home, supporting safety, comfort, and clinical stability throughout the night. Working independently with the support of a collaborative clinical team, you will monitor vital signs, administer medications as ordered, perform treatments and wound care, manage feeding tubes or other medical devices as needed, and promptly recognize and report changes in condition. You will follow individualized care plans, document all care and observations accurately in the electronic medical record, and communicate effectively with the supervising RN, physicians, and family members to ensure continuity of care. In this role, you will provide personal care and assistance with nighttime routines, positioning, and comfort measures, while maintaining infection control and safety standards. You will educate and support family or caregivers on basic care techniques within your scope of practice, promoting dignity and respect for the patient at all times.
