Job Overview We are seeking a dedicated and compassionate Long Term Care (LTC) Registered Nurse (RN) to join our team for immediate assignments.In this vital role, you will provide high-quality nursing care to residents in long-term care facilities, including nursing homes, assisted living communities, and memory care units.Your expertise will ensure residents receive personalized, comprehensive care that promotes their well-being, safety, and comfort.This position offers an exciting opportunity to make a meaningful difference in the lives of individuals requiring ongoing medical support and attention.Duties Conduct thorough patient assessments, including vital signs, physical examinations, and health history reviews to develop individualized care plans.Administer medications and injections accurately following HIPAA guidelines and medical documentation standards.Manage complex medical procedures such as IV infusion, catheterization, wound suturing, and ostomy care with aseptic technique.Monitor residents' conditions closely, recognizing signs of acute pain, infectious disease symptoms, or behavioral health issues to initiate timely interventions.Collaborate with interdisciplinary teams on discharge planning, case management, and coordination of services like hospice & palliative medicine or dialysis.Document all patient care activities meticulously using EMR (Electronic Medical Records) systems such as Cerner or Athenahealth while ensuring compliance with HIPAA regulations.Provide education and health coaching to residents and their families about medication management, nutrition, and chronic disease management.Skills Proven two year experience in long-term care settings such as nursing homes or assisted living communities with familiarity in geriatrics and dementia/dementia care.Strong knowledge of medical terminology, physiology, ICD coding (ICD-9/ICD-10), and diagnostic evaluation techniques.Skilled in emergency response procedures including airway management, basic life support (BLS), and critical care practices within Level I or Level II trauma center environments.Experience working with individuals with developmental disabilities or behavioral health needs is highly desirable.Proficiency in EMR/EHR systems like Cerner or eClinicalWorks; familiarity with laboratory experience and specimen collection is a plus.Ability to perform complex procedures such as tube feeding, venipuncture, phlebotomy, sterile processing, and gastrostomy care confidently.Excellent patient assessment skills combined with the ability to handle acute pain management scenarios and triage effectively.Join our team today to deliver exceptional nursing care that transforms lives!
