Job Description
The Admissions Coordinator/Social Work Assistant role is a professional position that supports the Allegan County Medical Care Community's Social Work and Admissions department by providing excellent communication with hospitals and other care providers to arrange short term rehabilitation and long term care admissions to the facility, complete admissions and financial paperwork with new residents and family members, introduce, welcome, and orient the new resident to the community and help with a smooth and transition, assist in handling questions/concerns that arise, assist with completing resident MDS interviews and assist with the discharge planning arrangements under the direction of the Social Services Director.Allegan County Medical Care Community has been a CMS 5-Star rated, Eden Alternative Community for many years and is located north of the town of Allegan.The community features a household/neighborhood lifestyle in a warm, family-focused atmosphere.Apply through Indeed with resume and visit our website and Facebook page at www.allegancountymcc.org and www.facebook.com/AlleganCountyMCC Hours:Full Time Position:Usual hours are Monday through Friday 8:30am-5:00pm Qualifications:Associate's degree or higher in a Social Services field Self-directed and self-motivated Strong organizational skills Extremely detail oriented Excellent verbal and written communication including telephone skills Knowledge of medical terminology and Electronic Medical Records Knowledge of Medicare, Medicaid, and other insurance plans Proficient in understanding advance directives, Powers of Attorney and Guardianship/Conservatorships.Excellent attendance Ability to organize and prioritize multiple tasks effectively Proficient computer skills Excellent assessment skills Outstanding customer service and personal skills Team player Ability to relate positively, effectively, and appropriately with hospitals, doctors, insurance companies, family members, and other staff Ability to speak about challenging topics such as finances, end of life care/hospice, advance directives, etc.Positive, friendly, hard-working attitude Compassionate with strong desire to work with long-term care residents and the elderly Ability to effectively work well in high stressful situations Ability to make sounds decisions as appropriate.Quick learn with a willingness to continue to learn.Education:Associate's degree or higher in a Social Services field. Preference will be given to candidate with prior SNF experience in admissions, social services, or administration.(Wage scale range is based on degree and years of experience.)
