VNS Health, a leading provider of home and community-based healthcare services in New York City, is seeking an Admissions RN for Hospice Home Care. In this role, you will be the first clinical point of contact for patients and families entering hospice services, providing comprehensive in-home assessments, education, and support during a critical time in their care journey. Drawing on your hospice and home care experience, you will evaluate patients’ clinical status, identify palliative care needs, and determine hospice eligibility in alignment with Medicare, Medicaid, and commercial insurance guidelines. You will develop and initiate individualized plans of care in collaboration with the interdisciplinary hospice team, including physicians, social workers, spiritual care providers, and home health aides, ensuring that patient goals, symptom management, and comfort measures are clearly defined and communicated. The Admissions RN will obtain and review medical histories, perform focused physical assessments, document findings in the electronic medical record, and coordinate timely start-of-care visits. You will provide clear education to patients and caregivers about hospice philosophy, services, medication management, equipment needs, and safety in the home, while addressing questions with empathy and cultural sensitivity. This position requires strong clinical judgment in pain and symptom management, excellent communication and teaching skills, and the ability to work autonomously in diverse home environments across New York City. You will collaborate closely with referral sources, including hospitals, skilled nursing facilities, and community providers, to facilitate smooth transitions to home hospice.
