Hourly Pay Range: $56.44 - $70.58 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors. Position Highlights Position: Advanced Practice Provider (APN/PA) - Primary Care - Internal Medicine Location: 9301 Golf Rd, Suite 302, Des Plaines Full Time/Part Time: Full Time, 40 hours per week Clinic Hours: Monday and Friday, 7am - 5pm. Tuesday, Wednesday, Thursday, 7am - 7pm. One evening requirement One Saturday per month required, 8am - 12pm Rotating Call Setting: Outpatient Job Description Exciting opportunity to join Endeavor Health in the Primary Care department. Under the direction of the collaborating physician(s), provides care to patients. Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions. Maintains records and writes progress notes on status, treatments and procedures. Follows patients through their course of treatment in the clinic and hospital setting. Collaborates with physicians and the health care team to achieve optimal patient outcomes. Prescribes pharmacologic and non-pharmacologic therapies as appropriate. Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum. Employs strategies, interventions and teaching to promote, maintain, and improve health. Provide comprehensive health assessments, differential diagnosis, and prescribe treatments under the protocol in the direct management of acute and chronic illness and disease.
