Communicate patient condition changes effectively to physicians, advocating for patient needs using structured communication methods like SBAR. Coordinate patient care collaboratively with healthcare team members and facilities to ensure seamless service delivery. Adapt positively to the dynamic healthcare environment, demonstrating professionalism and flexibility. Document patient care activities accurately at the point of care, including assessments, interventions, care plans, and physician communications using electronic medical records. Evaluate patient progress toward goals and update care plans accordingly. Develop age-appropriate, patient-centered care plans upon admission with active involvement of patients and families. Provide education tailored to patient and family needs, considering their educational level and cultural background. Engage in self-reflection to learn from experiences and improve practice.
