Job Description
Join a dedicated healthcare team as a Homecare Occupational Therapist (OT) providing compassionate care in a regional setting within the Midwest. This role involves developing personalized care plans after thorough patient evaluations to meet both patient and physician goals. You will work with home health and hospice patients, collaborating closely with interdisciplinary teams to ensure comprehensive care. Essential Duties and Responsibilities Evaluate patients for home health admission based on CMS or payer guidelines and agency policies. Perform initial and comprehensive assessments within required timeframes, documenting accurately. Create individualized, age-appropriate care plans focusing on self-management with measurable goals and interventions. Update care plans promptly to address new or resolved issues. Communicate progress and changes with physicians and care team members. Include patients and caregivers in care and discharge planning, tailoring education to their learning preferences and barriers. Manage care delivery efficiently, including scheduling and resource utilization. Complete all documentation timely and meet daily productivity goals. Supervise Certified Occupational Therapy Assistants (COTA) and homecare assistants as applicable, ensuring alignment with care plans. Collaborate with multidisciplinary teams through meetings and direct communication. Assist patients in obtaining necessary equipment and initiate discharge planning from admission. Provide treatments following physician orders and agency protocols, adhering to infection control standards. Education Graduate of an accredited Occupational Therapy program is required. Experience Minimum one year experience in a JCAHO-accredited hospital. At least one year in an acute rehabilitation facility or eq
