Location: Alameda, CA Schedule: Monday – Friday, 8:00 a.m. – 5:00 p.m. Salary Range: $75K to $84K a year Position Summary We’re hiring an Associate Mental Health Therapist to join our Alameda-based IBH program. This role is ideal for registered associates seeking structured supervision, hands-on experience, and meaningful client interaction in a team that values collaborative care across systems. Key Responsibilities Care Coordination & Program Support Deliver in-person and telehealth-based care coordination for individuals with behavioral health and social support needs Conduct comprehensive client assessments and contribute to individualized care planning Facilitate warm handoffs and referrals to behavioral health, medical, housing, and community-based resources Monitor engagement and support follow-up care to maintain service continuity Collaborate with internal care teams and external partners to address client needs holistically Participate in multidisciplinary team meetings, case reviews, and service planning sessions Document services and progress accurately in the electronic health record (EHR) Assist with data collection and service tracking to meet reporting requirements Compliance & Training Ensure compliance with Medi-Cal, HIPAA, and internal documentation protocols Attend required trainings, community forums, and IBH-related meetings Support program evaluation activities by monitoring outcomes and service delivery Contribute to outreach, resource navigation, and sustainability efforts Requirements Registered Associate Clinical Social Worker (ASW/ACSW) in the State of California Bachelor’s
