Chronic Care Staffing is Hiring a Bilingual Care Coordinator - LPN/LVN! Location: Remote (MST or PST Time Zones Only) Schedule: Monday-Friday Pay Rate: $24-$27/hour Employment Type: Full-Time Overview Chronic Care Staffing is seeking a compassionate and highly organized Bilingual Care Coordinator - LPN/LVN to support patients managing chronic health conditions through proactive outreach, education, and care coordination. This fully remote opportunity is ideal for bilingual nurses who want to transition from bedside care into a telephonic care management environment while maintaining meaningful patient relationships and improving long-term health outcomes. In this role, you will act as a trusted advocate for patients, helping them navigate Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs while collaborating with providers and care teams to ensure continuity of care. Responsibilities Conduct outbound patient outreach calls in both English and Spanish. Perform health risk assessments and support CCM and RPM activities. Document all patient interactions accurately within Electronic Health Record (EHR) systems. Educate patients and families on chronic disease management and treatment adherence. Identify gaps in care and escalate concerns appropriately to providers or internal teams. Coordinate care plans and maintain consistent patient follow-up schedules. Connect patients with community resources and healthcare support services. Maintain HIPAA compliance and confidentiality standards at all times. Collaborate with providers and care teams to improve patient engagement and outcomes Requirements Must Have Active, unencumbered LPN/LVN license (California or compact state accepted). Residency within the MST or PST time zone. Professional fluency in both English and Spanish. Current Basic Life Support (BLS) certification. Strong proficiency with EHR systems and Google Suite. Dedicated, HIPAA-compliant home office with reliable high-speed internet and dual-monitor setup. Excellent clinical reasoning, communication, and patient engagement skills. Nice to Have Previous experience in care coordination, CCM, TCM, or remote patient monitoring programs. Experience educating patients and families on chronic disease management. Familiarity with CMS compliance standards, quality measures, and healthcare documentation requirements.
