Job Description
Hours : 40 hours per week. Work schedule is Monday, Tuesday, Thursday, and Friday, 8:30 am to 5:00 pm or 9:00 am to 5:30 pm; Wednesday, 10:30 am to 7:00 pm Pay Rate : $28.85 to $29.13 per hour. There is a $5,000 bilingual differential add-on to annual salary for this position. Conduct case management, linkages and referrals to needed supports in the community. Act as a liaison between the clinic and community partners to facilitate referrals and coordination of care for existing clients. Assist the client in meeting client service plan goals, prioritizing areas of need, and steps needed to reach goals established, as well as identified areas of strength. Engage and follow up with disengaged clients. Additional Qualifications and Skills Bilingual Spanish is required for this position At least one year of related experience and/or training Must be able to work in a fast-paced and collaborative environment Proficient computer skills, including MS Office Able to pass criminal/background clearance CRF inspires and mentors the next generation of behavioral health professionals! Our Adult Outpatient Programs (Areta Crowell Wellness Recovery Center, Douglas Young Wellness Recovery Center, Heartland Wellness Recovery Center, Jane Westin Center, Maria Sardinas Wellness Recovery Center , and South Bay Guidance Center) provide behavioral health services including psychiatric and clinical assessment, treatment planning, ongoing medication services, individual and group counseling, case management, crisis intervention, mobile outreach, vocational support, and peer support. We deliver fully integrated services to adult clients diagnosed with a serious mental illness, as well as individuals with co-occurring, mental health, and substance use disorders. All of our outpatient programs also provide full-time walk-in services. Our programs utilize evidence-based practices to assist our clients in reaching their goals and to live the highest quality of life while managing their mental illness. CRF staff members work together to meet the needs of San Diego County's residents who are experiencing mental/behavioral health challenges.
