Description Care CoordinatorLocation: Wiscasset/Bath, Maine (Home- and community-based)Employment Type: Full-timeSalary: $52,000/yearDate Posted: April 9, 2026About the RoleClarvida is seeking a Care Coordinator to provide Targeted Case Management (TCM) and Behavioral Health Home (BHH) services to children and adults with behavioral or mental health diagnoses.
