Care Coordinator

Job Title:Care Coordinator Location:DFW Texas (Mobile Wound Care - Field-Based Coordination)Job Type:Full-TimeCompensation:$22-$25 per hour (based on experience) Job Summary We are seeking a dedicated and organized Care Coordinator to support our Mobile Wound Care team across Texas.This role is essential in coordinating patient care, scheduling services, communicating with facilities and providers, and ensuring a smooth continuum of care for our patients.The ideal candidate is detail-oriented, compassionate, and experienced in healthcare coordination or clinical support.Key Responsibilities Coordinate and schedule patient visits for wound care providers across multiple facilities Serve as a primary point of contact for patients, families, facilities, and clinical staff Communicate effectively with nursing facilities, home health agencies, and provider teams Ensure accurate documentation and maintenance of patient records in EMR systems Verify patient demographics, insurance information, and required authorizations as needed Assist with referrals, follow-ups, and continuity of care Monitor care plans and support providers with administrative coordination Address patient or facility concerns promptly and professionally Maintain compliance with HIPAA and company policies Qualifications High school diploma or equivalent required; Associate's or Bachelor's degree in healthcare or related field preferred Minimum 1-2 years of experience in healthcare coordination, medical office, or clinical support role Experience in wound care, home health, hospice, or long-term care preferred Strong communication and interpersonal skills Excellent organizational and multitasking abilities Proficiency with EMR systems and Microsoft Office Ability to work independently in a fast-paced environment Knowledge of insurance verification and healthcare workflows is a plus Benefits Competitive hourly compensation ($22-$25/hr) Full-time, stable position Supportive and collaborative work environment Opportunities for growth within a growing mobile wound care organization Work Environment This is a coordination-focused role supporting a mobile wound care model.The position may involve frequent communication with providers and facilities across Texas and requires flexibility and strong time-management skills.Job Type:Full-time Pay:$22.00 - $25.00 per hour Benefits:Flexible schedule Paid time off Professional development assistance Experience:Wound care:1 year (Required) Care Coordinator:1 year (Required) Work Location:In person.
