Job Title: Care Manager Location: New York, NY 10004 Duration: 02+ months Pay Range: $38 - $45 Job Description: Perform LTSS and SDOH assessments to evaluate member needs Coordinate care services including appointments, transportation, medications, and DME Document all interactions accurately in DCMS and related systems Coordinate and send plans of care (POCs) to acute care facilities for admitted members Notify primary care providers (PCPs) of patient admissions and discharges Perform timely and accurate data entry in the Disease Care Management System (DCMS). Conduct monthly member outreach calls in Spanish, ensuring HIPAA-compliant verification Bilingual in Spanish required.
