Mission Health, based in Asheville, NC, is seeking a compassionate and detail-oriented Care Manager Registered Nurse to join our Healthcare & Medical Services team. In this role, you will coordinate and manage the care of patients across the continuum, ensuring they receive safe, efficient, and patient-centered services. Working collaboratively with physicians, interdisciplinary team members, patients, and families, you will develop and implement individualized care plans, facilitate transitions of care, and advocate for appropriate clinical and community resources. The Care Manager Registered Nurse will assess patient needs, identify barriers to care, and use clinical judgment to support high-quality outcomes, reduced readmissions, and optimized length of stay. You will monitor patient progress, evaluate response to treatments, and adjust care plans as needed while maintaining accurate documentation within the electronic health record. This position involves regular communication with providers, social workers, therapists, and external partners to coordinate services such as home health, rehabilitation, and follow-up appointments.
