Job Overview Please fill the hiring form on Wellsky at :https://hh59012.clearcareonline.com/apply/ We are seeking a compassionate and dedicated Caregiver for Personal Care to provide exceptional support to individuals in need of assistance with daily activities and personal well-being.This role offers an energizing opportunity to make a positive impact on the lives of seniors, individuals with disabilities, or those recovering from illness.As a vital member of our caregiving team, you will deliver personalized care that promotes independence, dignity, and comfort in a variety of settings including private homes, assisted living facilities, group homes, and hospice environments.Your proactive approach and caring nature will help create a safe, nurturing environment for those you serve.Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor patients' health by taking vital signs such as blood pressure, pulse, and temperature; report any concerns promptly Support medication administration following care plans and HIPAA regulations to ensure privacy and safety Prepare nutritious meals tailored to dietary needs and assist with meal planning and cooking as required Maintain cleanliness of living spaces through regular cleaning tasks to promote hygiene and comfort Utilize equipment such as Hoyer lifts or transfer belts to safely assist with heavy lifting or transfers Observe patients closely for changes in condition or behavior; document observations accurately for care plans and team communication Support patients with cognitive impairments such as Alzheimer's disease or dementia through memory care techniques and patient engagement strategies Follow all safety protocols including CPR certification procedures and emergency response plans during incidents or health crises Collaborate with healthcare professionals, social workers, and family members to ensure comprehensive patient support in home health or hospice settings Skills Proven experience in home & community care, assisted living, nursing homes, or long-term care environments Strong knowledge of patient observation, vital signs monitoring, medication administration, and care planning Certifications such as First Aid, CPR, HIPAA compliance training preferred but not required; willingness to obtain necessary certifications is valued Experience working with individuals with developmental disabilities, autism spectrum disorder, or cognitive impairments like Alzheimer's or dementia care Ability to perform heavy lifting safely using equipment like Hoyer lifts; physical stamina for long shifts involving mobility assistance Excellent communication skills for working effectively with patients, families, and healthcare teams Compassionate attitude combined with patience and resilience to handle challenging situations calmly and professionally Familiarity with home health procedures including social work coordination and hospice care practices Join us in delivering heartfelt caregiving that transforms lives!
