Job Overview We are seeking compassionate, reliable Nurses and Caregivers to provide high-quality in-home support to individuals and families across Central New York and the Finger Lakes regions. In this role, you will deliver personalized care in private home settings. Assignments may include support for older adults, individuals with memory impairment, and those needing assistance with daily living. This opportunity is ideal for professionals who value flexibility, meaningful client relationships, and independence with professional support. Duties & Responsibilities Responsibilities may vary by assignment but can include: Provide assistance with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support Monitor and report changes in client condition or behavior Support clients with memory care needs, including Alzheimer’s and dementia Assist with meal preparation and nutritional support Provide companionship and emotional support Support safe transfers and mobility (including use of assistive devices such as Hoyer lifts when trained) Maintain a clean and safe home environment related to client care Document care activities as required by the care plan Collaborate with families and care teams to support continuity of care For Registered Nurses (RN), duties may additionally include: Medication administration per care plan Skilled nursing tasks within scope of license Clinical observation and reporting Experience & Qualifications Prior experience in home care, assisted living, nursing homes, or community-based care preferred Demonstrated knowledge of ADL support and client safety Experience supporting individuals with dementia or memory impairment strongly valued Current certification/licensure as applicable (RN, LPN, CNA, HHA, PCA) CPR and First Aid certification preferred Strong communication skills and professional reliability Ability to safely assist with transfers and mobility support Commitment to maintaining client dignity and confidentiality All caregivers are subject to screening, background checks, and reference verification prior to placement.
