Job Description
Position Summary: The Case Manager plays a vital role in supporting children and families involved in the child welfare system. This position ensures the safety, well-being, and permanency of children who have been removed or are at risk of removal from their homes due to abuse, neglect, or abandonment. Case Manager is responsible for implementing the Florida Child Welfare Practice Model, focusing on family-centered practices, trauma-informed care, and community-informed service delivery. Essential Job Functions: Assess the needs and strengths of children and family Develop Case Plans in collaboration with the children and family Conduct monthly Case Plan reviews Document all services and activities timely in accord with certification, licensing, Medicaid, accreditation standards and agency policy and procedure Conduct individual case management sessions, as needed Perform activities on behalf of the children and family, including attendance at external appointments (e.g., court appearances, etc.), as needed Provide referrals and linkage for the children and family, as needed Prepare Judicial Reviews Social Study report (JRSSR) and Progress Reports, as required, for assigned child and family for courts, probation officers, caseworkers, etc. Coordinate child and family care with the treatment team and with external providers Responsible for the development of the case plan with other treatment team members Provide, arrange and coordinate transportation for child and family, as necessary Participate in case conferences Participating in multidisciplinary team meetings as required Will utilize Stabilify Parent Portal or other data systems required to support the case management role.
