About Us Boston Rehabilitation Center/Shore Point Recovery is a premier behavioral health facility dedicated to transforming lives through compassionate, individualized care.Our Wareham facility is a 35-bed detox and residential treatment center, providing 24/7 support for individuals recovering from substance use disorders.Position Overview We are seeking dedicated and detail-oriented Case Managers to support clients in our 40-bed detox and residential program.Case Managers play a vital role in coordinating care, advocating for clients, and ensuring they have access to the resources needed for a successful recovery.This is a full-time, onsite position, SUNDAY-THURSDAY, 8am-4:30pm position.Key Responsibilities Conduct comprehensive client assessments and develop individualized treatment plans. Coordinate discharge planning and aftercare services, ensuring continuity of care. Advocate for clients and assist them in accessing community resources, including housing, employment, and continued treatment. Collaborate with clinical and medical teams to support client's treatment goals. Facilitate case management sessions and provide ongoing support to clients throughout their recovery journey. Maintain accurate and timely documentation in compliance with regulatory standards. Monitor client progress and adjust care plans as needed. Provide education on relapse prevention, life skills, and available support services.Qualifications Experience in substance abuse treatment or behavioral health settings required. Bachelor's degree in social work, psychology, counseling, or a related field preferred. Knowledge of case management principles, discharge planning, and community resources. Strong communication, organizational, and problem-solving skills. Ability to work SUNDAY-THURSDAY, 8A-4:30P hour shift in a fast-paced environment.
