Position Overview AKMFT is seeking a compassionate, organized full‑time Case Manager / Front Desk Relief to join our collaborative mental‑health team. This role provides coordinated, client‑centered support that enhances access to therapy, strengthens continuity of care, and removes practical barriers to treatment. The Case Manager serves as a communication bridge between clients and therapists, supports administrative and care‑coordination processes, and helps ensure clients are connected to services that promote mental‑health stability and whole‑person wellness. This position also provides front desk relief coverage as needed to ensure a welcoming, accessible, and responsive clinic environment. Respond to inquiries related to scheduling, referrals, treatment logistics, and resources. Triage client concerns and communicate clinically relevant updates to therapists promptly. Support continuity of care by reinforcing treatment plans and care coordination. Intake Paperwork Guidance & Documentation Support Review intake documents with clients using clear, accessible, and trauma‑informed communication. Assist clients with paperwork when literacy, cognitive, emotional, or situational barriers exist. Ensure all required documentation is complete, accurate, and ready for clinical review. Uphold confidentiality and HIPAA compliance. Ongoing Treatment Check‑Ins & Progress Monitoring Conduct structured treatment check‑ins every three months. Assess client progress, functioning, and emerging needs. Identify barriers to engagement and collaborate with the care team on solutions. Document findings accurately in the electronic health record. Care Transitions & Provider Coordination Support clients navigating concerns related to provider fit or treatment effectiveness. Coordinate transitions, warm handoffs, referrals, and transfer of records as needed. Provide logistical and emotional support during changes in care. External Services & Follow‑Up Coordination Assist clients with referrals to medical, psychiatric, and community providers. Educate clients on what to expect from additional services. Track referrals and confirm engagement with recommended care. Follow‑Up With Clients Who Discontinue Services AMA Conduct timely, non‑judgmental outreach to clients who discontinue services against medical advice (AMA). Assess safety needs and communicate concerns to the clinical team when indicated. Offer options for re‑engagement, alternative providers, and community or crisis resources. Document all outreach efforts and outcomes thoroughly. Front Desk Relief Responsibilities Provide front desk coverage during staff absences, high‑volume periods, or coverage gaps. Greet clients, visitors, and vendors in a professional, welcoming, trauma‑informed manner. Manage incoming calls, screen and route messages appropriately. Support client check‑in/check‑out and appointment scheduling and confirmations. Maintain front desk organization, confidentiality, and professionalism. Assist with general administrative tasks including filing, scanning, and data entry. Perform other duties as assigned to support clinic operations and client access to care. Qualifications Bachelor’s degree in social work, psychology, human services, or related field (or equivalent experience). Experience in mental‑health, healthcare, or social‑service settings preferred. Strong organizational, communication, and problem‑solving skills. Ability to collaborate effectively within a multidisciplinary team. Knowledge of HIPAA, confidentiality standards, and trauma‑
