CASE MANAGER MEDICAL

The SCI Medical Case Manager is responsible for assisting with the coordination of all system services required for an organized, multidisciplinary team approach to patient care. Additionally, this role assists in assuring quality, cost- efficient care for the rehab patient population. MINIMUM EDUCATION: Associate degree required MINIMUM WORK EXPERIENCE Minimum of 1- 3 years’ experience in health, social or human service or community-based setting KNOWLEDGE & SKILLS: Must have strong organizational skills Excellent oral and written communication skills required Knowledge of basic computer operations required REQUIRED LICENSES, CERTIFICATES, REGISTRATIONS: None Location: SHS · SCI SCI SP HEALTH WORKS Schedule: Full Time, Days, 8:30am-5:00pm
