Header* Job Summary* Join our compassionate team as a Certified Nursing Assistant (CNA) providing in-home care that makes a real difference! In this vital role, you will deliver personalized, high-quality support to clients in their homes, promoting independence and comfort. Your energetic and caring approach will help individuals with diverse needs, including seniors, those with disabilities, or individuals requiring specialized dementia and Alzheimer's care. This position offers the opportunity to work closely with clients to enhance their quality of life through attentive patient care and compassionate assistance. All roles are paid positions, emphasizing the value of your dedication and skills. Header* Duties* Provide direct patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices. Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation within Electronic Health Record (EHR) systems while ensuring HIPAA compliance. Administer medications and manage treatments following prescribed medical terminology and protocols under supervision or as trained. Support clients with memory care needs, including dementia and Alzheimer’s care, creating a safe environment tailored to their unique requirements. Assist with medical procedures such as catheter care, wound dressing, and hospice support when necessary. Prepare nutritious meals and assist with feeding while maintaining cleanliness through laundry and light cleaning duties to promote a healthy living space. Document patient progress thoroughly in medical records, ensuring compliance with healthcare standards and confidentiality regulations. Collaborate effectively with healthcare teams using EMR systems to update patient information and coordinate care plans efficiently. Header* Qualifications* Valid CNA certification recognized by state licensing boards. Proven experience in home health or long-term care environments; nursing home experience is highly valued. Strong knowledge of medical terminology, medication administration, and vital signs monitoring. Experience working with individuals with developmental disabilities or dementia is preferred. Ability to operate medical equipment such as Hoyer lifts, EHR systems, and EMR platforms confidently. CPR certification is required; additional certifications like hospice care or catheter care are a plus. Excellent patient observation skills coupled with compassionate caregiving abilities. Ability to work independently in home settings while adhering to safety protocols and HIPAA regulations.
