Job Description
Overview We are seeking a compassionate and dedicated to join our team in providing exceptional care and support to individuals in need of assistance. Whether working in assisted living facilities, nursing homes, or home health environments, you will play a crucial role in enhancing quality of life through attentive caregiving and professional medical support. Duties Observe and monitor patients’ conditions, including vital signs and behavioral changes, to ensure safety and well-being Assist with Activities of Daily Living (ADLs), such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts Administer medications accurately following medical documentation protocols while maintaining HIPAA compliance Support patients with dementia or Alzheimer's disease through specialized memory care techniques and compassionate engagement Perform catheter care, wound management, and hospice care as needed to promote comfort and health Document medical records meticulously using Electronic Health Record (EHR) systems and update patient charts regularly Assist with meal preparation, feeding, laundry, cleaning, and other home care tasks to maintain a safe and hygienic environment Collaborate with healthcare teams by communicating patient status, changes in condition, or concerns promptly and professionally Experience Prior experience working in nursing homes, assisted living facilities, or home health environments preferred Proven ability to work with individuals with developmental disabilities or dementia-related conditions Knowledge of medical terminology and procedures related to patient observation and care management Certification in CPR (Cardiopulmonary Resuscitation) required; additional certifications such as hospice care or catheter care are a plus Experience operating EMR (Electronic Medical Records) or EHR systems for accurate medical documentation Long-term care experience demonstrating stability and commitment in caregiving roles Ability to handle sensitive situations with empathy while adhering to HIPAA regulations and confidentiality standards Join us in delivering compassionate care that transforms lives!
