Responsibilities: Receives referrals from the Access team with identified client needs such as Medicaid enrollment, Veteran Affairs authorizations, and determination of need for interagency services Assists clients with Medicaid, Veteran Affairs, or other payer source enrollment Completes care coordination assessment understanding the physical, financial and emotional needs of the client, which becomes a strategy for successful transition into community. Serves as the contact point, advocate and informational resource for consumers, care team, law enforcement, family/caregiver(s), payers and community resources. Works with consumers to plan and monitor care: Assess consumer's unmet health and social needs. Contributes to the development of a care plan/treatment plan with the consumer, family/caregiver(s) and providers Supports the development of and complete ident
