Our mission is to innovate health solutions that deliver maximum value and impact. Job Summary Acentra Health is seeking a Clinical Social Worker (LCPC/LCSW) (Remote) to join our growing team. The role involves conducting comprehensive, needs‑based eligibility assessments for individuals applying for Medicaid‑funded personal care services delivered in private homes, adult care settings, or supervised living environments. You will evaluate level‑of‑care criteria to support informed, person‑centered determinations that enable individuals to remain in or safely transition back to home and community‑based settings. Job Responsibilities Conduct a comprehensive assessment with youth, guardian, and treatment team members. Determine whether the beneficiary meets the conditions and criteria using state‑approved standardized assessment tools for specific services. Ensure services provided on a needs basis in quantities appropriate to the beneficiary’s unmet need based on severity, functioning, physical, or cognitive impairment. Maintain privacy and dignity of individuals receiving assessment at the highest standards. Conduct new, expedited, annual, and change‑of‑status assessments within established timeframes. Include an interview with family members and informal caregivers. Attend and actively participate in staff meetings and conduct case consultations/peer reviews/internal auditing as assigned. Read, understand, and adhere to all corporate policies including HIPAA and its Privacy and Security Rules. Required Qualifications Active and unrestricted Licensed Clinical Social Worker (LCSW) or Licensed Clinical Professional Counselor (LCPC) license for the state of Maine. Graduate from an accredited Master’s Degree Program. Minimum of two years’ experience in behavioral/mental healthcare. Minimum of one year of assessment experience. Strong clinical assessment and critical thinking skills. Medical record abstracting skills. Preferred Qualifications Knowledge of the organization of medical records and medical terminology. Excellent verbal and written communication skills. Ability to work in a team environment. Flexibility and strong organizational skills. Strong working knowledge of Microsoft Office Suite, including Word, Excel, Outlook, and Teams.
