Facility: Mercy Hospital of Buffalo Shift: Shift 1 Status: Full Time FTE: 1.000000 Bargaining Unit: ACE Associates Exempt from Overtime: Exempt: Yes Work Schedule: Days and Evenings Hours: primarily 8-4 PM with flexibility for department needs Summary In concert with the Director, Care Management and/or Manager, Care Management, this position is responsible to supervise the day-to-day operations of the Care Management Department to ensure consistent, effective and efficient site-specific medical management, discharge planning and care coordination for patients across our continuum. Assists in achieving site specific and system wide discharge planning performance metrics. Assists the Manager in providing clinical oversight to the Care Management staff. Serves as subject matter expert and coach in terms of effective and timely discharge planning, care coordination including identification and resolution of patient and family concerns, insurance issues and/or discharge to other appropriate levels of care. Responsibilities EDUCATION Bachelors in Nursing required Registered RN in New York State Specialty certification preferred (i.e. CCM ) EXPERIENCE Three (3) years care management/medical management experience Demonstrated track record of success/accountable for site/system wide clinical, quality and financial results in an acute care or managed care environment One (1) to two (2) years Management/supervisory experience preferred KNOWLEDGE, SKILL AND ABILITY Demonstrated ability to think autonomously, make strategic decisions and balance multiple priorities Excellent critical thinking skills, complex problem solving and analytical/statistical skills Extensive knowledge of medical management, utilization, discharge planning principles /practice Extensive knowledge and fluency of NYS DOH , CMS, JCAHO standards , third party regulations and compliance standards Stellar interpersonal/intrapersonal relationships with interdisciplinary teams Recognized and viewed as subject matter expert on care management, care coordination, disease management, discharge planning, utilization review for their respective site WORKING CONDITIONS Routine travel between acute care facilities may be required Flexibility in scheduling and on site assignment Normal heat, light space, and safe working environment; typical of most office jobs
