Position Overview We are seeking an experienced and compassionate Clinical Supervisor / Team Manager to lead hospice clinical operations and oversee an interdisciplinary care team. This role is responsible for ensuring the delivery of high-quality, patient-centered end-of-life care while maintaining compliance with all regulatory and organizational standards. The ideal candidate will have prior hospice leadership experience, a strong understanding of regulatory requirements, and a passion for guiding teams to deliver exceptional care. Key Responsibilities Supervise and support clinical staff, including nurses, social workers, and other care team members Coordinate and oversee daily clinical operations to ensure timely and effective patient care delivery Review patient referrals, assess care needs, and assign appropriate clinical resources Guide clinicians in developing and implementing individualized care plans Monitor and evaluate staff performance, providing coaching, feedback, and professional development Conduct regular chart audits and ensure accuracy, quality, and compliance of clinical documentation Ensure adherence to all federal, state, and local hospice regulations and standards Facilitate interdisciplinary team meetings and promote effective communication across departments Participate in hiring, onboarding, and training of clinical staff Support quality improvement initiatives and performance improvement programs Address patient, family, and staff concerns with professionalism and empathy Collaborate with leadership to optimize staffing, workflow, and operational efficiency Qualifications Active Registered Nurse (RN) license (or applicable clinical licensure as required) Bachelor's degree in Nursing or related healthcare field (BSN preferred) Minimum 2+ years of supervisory or management experience in hospice or home health Strong knowledge of hospice philosophy, care practices, and regulatory requirements Demonstrated leadership, team management, and decision-making skills Excellent communication, organizational, and interpersonal abilities Experience with electronic medical records (EMR/EHR) systems Ability to manage multiple priorities in a fast-paced clinical environment Key Competencies Leadership & team development Clinical excellence & patient-centered care Regulatory compliance & quality assurance Critical thinking & problem-solving Effective communication & collaboration Why Join Opportunity to lead a dedicated hospice care team Meaningful work supporting patients and families during critical moments Collaborative and mission-driven environment Career growth within clinical leadership
