Virtual: This role enables associates to work virtually full-time, except for required in-person training sessions, providing maximum flexibility and autonomy. This approach promotes productivity, supports work-life integration, and ensures essential face-to-face onboarding and skill development. Alternate locations may be considered if candidates reside within a commuting distance from an office. Schedule: 10AM-7PM CST or 9AM-6PM CST. Monday through Friday, no weekends or holidays required. The Community Engagement Navigator- Behavioral Health is responsible for engaging with members to remove barriers to Social Determinants of Health (SDOH) and acting as the communication liaison between the company and referral sources. How you will make an impact: Engage with members who have been identified as possessing significant SDOH barriers. Work with the member to establish goals and next steps for addressing SDOH. Support member in selecting provider or programs most appropriate for supporting their needs. Interface with HCM to ensure that the individualized plan is aligned with existing or needed case management efforts. Act as a liaison between referral sources and the health plan. Associates in this role are expected to have the ability to multi-task, including handling calls, texts, facsimiles, and electronic queues, while simultaneously taking notes and speaking to customers. Proficient in maintaining focus during extended periods of sitting and handling multiple tasks in a fast-paced, high-pressure environment; strong verbal and written communication skills, both with virtual and in-person interactions; attentive to details, critical thinker, and a problem-solver; demonstrates empathy and persistence to resolve caller issues completely; comfort and proficiency with digital tools and platforms to enhance productivity and minimize manual efforts. Associates in this role will have a structured work schedule with occasional overtime or flexibility based on business needs, including the ability to work from the office as necessary. Performs other duties as assigned. Minimum Qualifications: BA/BS degree in Public Health, Social Work, Rehabilitation Counseling or related field and a minimum of 2 years experience with marketing, job development, job placement or social work; or any combination of education and experience which would provide an equivalent background. Travels to worksite and other locations as necessary. Preferred Skills, Capabilities and Experiences: Knowledge of Voc Rehab preferred. Previous Behavioral Health experience preferred. Previous experience working with SDOH preferred. Previous experience with using electronic medical records systems (EPIC). Previous experience with using MS Teams, Excel and/or SmartSheet. For candidates working in person or virtually in the below locations, the salary* range for this specific position is $27.75 to $41.64. Location(s): Colorado and Illinois.
