Job Description
Join our dedicated team as a Community Health Worker and become a vital link in promoting health, wellness, and equitable access to healthcare services within diverse communities.Together with our team you will serve Cedar, Dixon, Thurston and Wayne counties to implement, coordinate, and ensure delivery of services for one or more public health programs which may include, but not limited to chronic disease prevention, and other related public health programs.Engage with community members to assess their health needs and connect them with appropriate resources Provide education on topics such as nutrition, healthy habits, wellness and preventive health measures Conduct outreach activities including outreach, health fairs, classes and public speaking engagements to raise awareness about health issues Utilize motivational interviewing techniques to support behavior change and enhance motivation for healthier lifestyles Collect and document data related to community health trends, service utilization, and client progress using standardized tools Assist individuals with navigating healthcare systems such as Medicare or Medicaid to ensure they receive necessary benefits Collaborate with healthcare providers, social workers, partners and community organizations to coordinate care plans and support services Prior experience or training in nursing, social work, public health, or related fields is highly desirable Experience working with individuals with disabilities or vulnerable populations is preferred Proven ability to communicate clearly through public speaking or teaching engagements Familiarity with data collection methods and documentation standards for health programs Decision making skills and able to collaborate with partners as needed to meet deliverables of grants Demonstrated commitment to community service with excellent interpersonal skills and cultural sensitivity This role is an excellent opportunity for motivated individuals passionate about improving community health outcomes.Join us in making a difference--apply today!
