Job Description
Alignment Health is breaking the mold in conventional health care, committed to serving seniors and those who need it most: the chronically ill and frail. It takes an entire team of passionate and caring people, united in our mission to put the senior first. We have built a team of talented and experienced people who are passionate about transforming the lives of the seniors we serve. In this fast-growing company, you will find ample room for growth and innovation alongside the Alignment Health community. Working at Alignment Health provides an opportunity to do work that really matters, not only changing lives but saving them. Together. This is a field remote position. Candidates must live in San Joaquin, Merced or Stanislaus Counties. The Community Outreach Representative serves as the frontline brand ambassador for Alignment Health, generating qualified leads and building meaningful connections across assigned community territories. This role works in close partnership with licensed sales agents, marketing, and compliance teams to convert community engagement into measurable enrollment outcomes. The representative directly supports plan growth by cultivating trusted relationships with seniors, underserved populations, venue partners, and community organizations. This role is critical to the organization because grassroots outreach remains one of the highest-converting and most cost-effective channels in Medicare Advantage member acquisition and retention. Job Duties/Responsibilities: Generate and Qualify Leads Through Community Engagement Drive consistent lead production by executing a structured outreach calendar that includes health fairs, senior center visits, retail events, food banks, churches, and community organizations. Meet or exceed weekly and monthly lead generation targets, maintaining accurate and timely records in Salesforce to ensure pipeline visibility for the sales team. Build and Sustain Community Venue Partnerships Identify, cultivate, and maintain productive relationships with venue partners — including senior living facilities, libraries, faith-based organizations, and retail locations — that generate a reliable flow of prospecting opportunities. Develop repeatable partnership frameworks that produce sustained lead volume across the assigned territory. Plan, Coordinate, and Execute Outreach Events Lead end-to-end logistics for community and educational events, including scheduling, venue setup, materials preparation, and staffing coordination. Deliver monthly event plans on time, ensuring all events are registered, documented, and executed in alignment with CMS marketing guidelines and internal compliance standards. Educate Prospects And Members On Plan Benefits Present Alignment Health plan benefits, services, contracted providers, and eligibility in a clear, culturally competent, and compliant manner to prospective and current members. Conduct new member orientation sessions and ongoing education events to drive plan understanding, satisfaction, and retention. Support Licensed Agents in Lead Conversion and Enrollment Coordinate closely with assigned sales agents to hand off qualified leads, support advertised and non-advertised sales presentations, and participate in enrollment meetings where appropriate. Track lead-to-enrollment conversion activity in Salesforce and align outreach strategies to local market sales goals. Maintain Compliance with CMS and Internal Marketing Standards Distribute only approved marketing and event materials, and partner with Marketing and Compliance teams on the development of community-facing collateral. Escalate any compliance concerns related to event conduct, venue arrangements, or member communications promptly and through appropriate channels. Track Activity, Report Results, and Optimize Performance Maintain daily Salesforce updates, submit weekly activity summaries, and provide a monthly calendar of planned events to management. Use reporting data to identify top-performing venues and event types, continuously refining outreach strategy to improve lead quality and event ROI.
