Job Description
JOB SUMMARY: The Continuous Improvement Coordinator supports performance and quality improvement initiatives by working closely with clinic sites, health plans, and patients. This role is responsible for chart review and coding, patient outreach, scheduling appointments, and supporting audit and reporting processes. The Coordinator plays a key role in helping the department meet HEDIS and Pay-for-Performance (P4P) goals through efficient communication, accurate data handling, and effective coordination. ESSENTIAL DUTIES AND RESPONSIBILITIES: Review and understand Pay-for-Performance (P4P) programs for various health plans. Scrub patient charts and enter CPT codes accurately to ensure correct documentation and data reporting. Make high volumes of outbound calls to patients to schedule appointments for clinic visits, with a daily goal of 75 calls and 14 scheduled appointments. Scrub health plan reports to identify care gaps related to HEDIS measures and prepare lists for patient outreach. Work with health plans on audit requests by reviewing and submitting requested documentation. Support clinic sites during events and initiatives aimed at improving HEDIS metrics. Track and update appointment and outreach data using Excel and internal systems. Maintain regular communication with clinic teams, patients, and leadership regarding outreach progress and care gap closures. Perform all other duties as directed either formally or informally, verbally or in writing. SUPERVISORY RESPONSIBILITIES: No direct supervisory responsibilities. KNOWLEDGE, SKILLS AND ABILITIES: Strong organizational skills and attention to detail; able to manage multiple responsibilities efficiently. Proficient in electronic health record (EHR) systems; NextGen experience preferred. Skilled in Microsoft Office Suite, particularly Excel, Outlook, and Word. Excellent verbal and written communication skills. Demonstrates high productivity and professionalism when handling a large volume of outbound phone calls. Effective interpersonal skills and a collaborative, team-oriented approach. Maintains a high level of professionalism in conduct, appearance, and communication. Flexible and able to adapt to changing tasks and priorities as needed. EXPERIENCE AND EDUCATION: High School Diploma or equivalent. Minimum of six (6) months of experience in a similar clerical role with solid knowledge of an electronic health record (EHR) system; preferably in a healthcare setting and Nextgen. Knowledgeable in healthcare-related incentive programs such as pay for performance (P4P).
