HōttoCare is redefining dementia care by delivering compassionate, Medicare-covered services directly in the home. We support both individuals living with dementia and their caregivers, recognizing that the best care addresses the needs of both. This clinical role will work within a thoughtfully designed ecosystem of team, standardized processes, and technology. Responsibilities Fully remote, clinical position 80% time: Care plan development 10% time: Supporting/educating care navigators 5% time: Behavioral health video visits with family caregivers 5% time: Care coordination with external healthcare teams Flexible scheduling - work at times that fit your day Minimum time commitment of 8 hours per week Qualifications Valid CA license and Nurse Practitioner licensure 5+ years of full-time clinical experience 2+ years of dementia care, senior/geriatric care, or complex care experience Experience and love of behavioral health a plus Experience with motivational interviewing skills a plus Comfort with technology and communicating independently in an asynchronous team environment
