Job Description
Job Summary Love Plus Home Health Care is seeking compassionate, dependable, and motivated Direct Support Professionals (DSPs) – Individual Support Services to provide community-based support to individuals with behavioral health, developmental, and/or intellectual disabilities. This role focuses on promoting independence, community integration, skill-building, emotional wellness, and overall quality of life while supporting individuals within their homes and communities. Our DSPs play an important role in empowering individuals to achieve personal goals, strengthen daily living skills, and remain safely integrated within their communities. Duties Provide individualized support services in home and community settings Assist individuals with developing and strengthening independent living skills and daily routines Support community integration and participation in meaningful activities Promote self-advocacy, confidence, social development, and emotional wellness Assist individuals with goal achievement as outlined in person-centered plans Support individuals with maintaining a safe, clean, and healthy living environment Provide behavioral and emotional support using positive, recovery-oriented approaches Maintain appropriate professional boundaries and confidentiality in accordance with HIPAA guidelines Accurately complete required documentation, service notes, and incident reporting Communicate effectively with supervisors, care teams, families, and other professionals involved in services Observe and report significant changes, concerns, or safety issues to appropriate supervisory staff Transport individuals within the community as needed for appointments, activities, or daily living needs Qualifications & Skills Experience working with individuals with behavioral health, developmental, intellectual, or emotional support needs preferred High school diploma or equivalent required Valid driver’s license and reliable transportation required Ability to work independently and within a team environment Strong communication, interpersonal, and problem-solving skills Compassionate, patient, dependable, and professional demeanor CPR/First Aid certification preferred or willingness to obtain Ability to complete required trainings and documentation in compliance with Medicaid and agency standards Must successfully pass required background checks and onboarding requirements Compensation Competitive pay based on experience, qualifications, availability, and assigned services Opportunities for professional growth, training, and advancement Flexible scheduling opportunities may be available Join us in creating a supportive environment where every individual receives respectful, personalized care.
