FTE: 1.0, Shift: Day, Schedule: Business Hours Position Summary The Director of Case Management is responsible for leading a dynamic team of clinical professionals in the execution of exemplary cross continuum care coordination. This position is responsible for the development and implementation of the case management process; coordinating and supervising the duties of the clinical case managers and social workers; promoting quality patient care outcomes while supporting appropriate utilization/resources management along the continuum of care; and facilitating the multidisciplinary case management process by coordinating the development and implementation of tools and systems for effective, efficient case management. This individual works with market leaders, physicians, peers, community colleagues, system leaders and others to identify opportunities to continually improve patient care and services, and to execute collaborative practices that exemplify MultiCare Health System's commitment to patient centered care and community engagement. Hospital-based leaders may have a dual matrixed reporting structure.
