About Us: Santana Hills Healthcare Center is a 167-bed facility with 90 skilled nursing beds and 77 sub-acute beds, delivering compassionate, patient-centered care for rapid recovery in a central San Jose location. Position Overview: The Director of Rehabilitation (SNF) will play a pivotal role in overseeing and enhancing the rehabilitation services provided to our patients. This position is responsible for developing and implementing effective rehabilitation programs that meet the diverse needs of our patient population. The Director will lead a team of skilled therapists and support staff, ensuring high standards of care and compliance with regulatory requirements. Additionally, this role involves collaborating with other healthcare professionals to create comprehensive treatment plans that promote optimal recovery outcomes. Ultimately, the Director of Rehab will contribute to the overall mission of improving the quality of life for our patients through innovative and compassionate rehabilitation services. Oversee the daily operations of the rehabilitation department, ensuring efficient service delivery. Develop and implement rehabilitation programs tailored to the needs of individual patients. Supervise and mentor rehabilitation staff, providing guidance and support to enhance their professional development. Collaborate with interdisciplinary teams to create and review patient care plans. Monitor and evaluate the effectiveness of rehabilitation services, making adjustments as necessary to improve patient outcomes. Qualifications: Master's degree in Physical Therapy, Occupational Therapy, or a related field. Valid California state licensure as a Physical Therapist or Occupational Therapist. A minimum of 5 years of experience in a rehabilitation setting, with at least 2 years in a leadership role. Certification in a specialized area of rehabilitation is preferred Experience with electronic health record (EHR) systems and data management.
