Description Assess patients in either an inpatient or outpatient environment and craft personalized treatment plans Implement physical therapy interventions concentrating on mobility, strength, balance, and functionality Collaborate with nurses, physicians, case managers, and other rehab specialists to optimize patient outcomes Inform patients and their families about therapeutic goals, discharge strategies, and exercises for home Record evaluations, treatments, and progress while adhering to facility and regulatory protocols Engage in care team discussions and interdisciplinary meetings to ensure coordinated patient care Education Doctorate in Physical Therapy (DPT) or equivalent qualification from an accredited institution Certifications State licensure as a Physical Therapist CPR cert
