Description Conduct evaluations for patients in both inpatient and outpatient settings and tailor individual treatment plans Implement physical therapy interventions emphasizing mobility, strength, balance, and overall function Team up with nurses, physicians, case managers, and rehabilitation specialists to maximize patient outcomes Educate patients and their families about therapeutic objectives, discharge planning, and home exercises Maintain detailed documentation of evaluations, treatments, and progress adhering to facility and regulatory standards Engage in care team discussions and interdisciplinary meetings to guarantee well-coordinated care Education Doctor of Physical Therapy (DPT) or an equivalent degree from an accredited institution Certifications State licensure as a Physical Therapist
