Description Conduct thorough evaluations for patients in both inpatient and outpatient environments and create tailored treatment plans Deliver physical therapy practices emphasizing mobility, strength, stability, and functionality Engage with a multidisciplinary team of nurses, physicians, case managers, and other rehabilitation experts to maximize patient outcomes Inform patients and their families about therapeutic objectives, discharge processes, and home-based exercises Record evaluations, therapies, and progress in line with facility standards and regulatory demands Take part in interdisciplinary rounds and care conferences to uphold coordinated patient care Education Doctorate in Physical Therapy (DPT) or equivalent from a recognized institution Certifications Active state licensing as a Doctor of Physical Therapy CPR certification preferred <
