Description Conduct thorough evaluations in inpatient or outpatient settings to create personalized treatment plans Implement physical therapy interventions aimed at enhancing mobility, strength, balance, and overall function Engage with nurses, physicians, case managers, and rehabilitation professionals to maximize patient outcomes Educate patients and their families about therapeutic objectives, discharge planning, and home exercise regimens Maintain detailed documentation of evaluations, therapies, and patient progress per facility and regulatory standards Engage in interdisciplinary conferences and care team rounds to ensure seamless coordinated care Education Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution Certifications State licensure as a Physical Therapist Preferred CPR certifica
