Collaborate actively with nurses, physicians, case managers, and other rehabilitation professionals to elevate treatment outcomes. Educate patients and their families on therapeutic objectives, home exercises, and discharge planning. Thoroughly document patient evaluations, treatment progress, and outcomes, conforming to regulatory and facility standards. Participate in interdisciplinary conferences and care team rounds to guarantee coordinated and effective patient care.
