Description Conduct evaluations in both inpatient and outpatient settings to create personalized treatment plans tailored to each patient's needs Implement physical therapy techniques that aim at enhancing mobility, strength, balance, and overall functionality Work hand-in-hand with nurses, physicians, case managers, and other rehabilitation staff to maximize patient outcomes Guide patients and their families through the therapeutic process, including setting goals, planning discharge, and recommending home exercises Maintain accurate documentation of evaluations, treatments, and patient progress following facility and regulatory standards Engage in care team discussions and interdisciplinary meetings to ensure holistic and coordinated care for patients Education Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution <
