Description Conduct comprehensive evaluations in inpatient and outpatient settings while crafting personalized treatment plans Implement therapeutic interventions aimed at enhancing mobility, strength, balance, and overall function Collaborate with a multidisciplinary team including nurses, physicians, case managers, and rehab specialists to ensure optimal patient outcomes Educate patients and families about therapeutic aims, discharge processes, and home exercise programs Maintain accurate documentation of evaluations, treatments, and patient progress in compliance with regulatory standards Engage in collaborative care team rounds and interdisciplinary meetings for effective care coordination Education Doctorate in Physical Therapy (DPT) or equivalent from an accredited
