Description Conduct detailed evaluations for patients in both inpatient and outpatient environments and craft personalized treatment plans. Guide patients and their families through therapeutic goals and discharge strategies, ensuring clarity and support. Maintain comprehensive documentation of evaluations, treatments, and patient progress in line with both facility and regulatory standards. Engage in interdisciplinary meetings and care team rounds to promote cohesive and coordinated patient care. Education Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution.
