Description Conduct thorough evaluations of patients in either inpatient or outpatient settings and create tailored treatment plans Implement physical therapy interventions targeting mobility, strength, balance, and function Collaborate with nurses, physicians, case managers, and other rehabilitation professionals to enhance patient outcomes Guide patients and their families through therapeutic objectives, discharge planning, and home exercise programs Maintain accurate documentation of evaluations, treatments, and progress in compliance with all facility and regulatory standards Engage in care team rounds and interprofessional conferences to ensure patient-centered, coordinated care Education Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institutio
