Description Assess and evaluate patients in both inpatient and outpatient settings to formulate custom treatment plans Implement physical therapy interventions centered on mobility, strength, balance, and functional recovery Engage in collaboration with nurses, physicians, case managers, and interdisciplinary rehab professionals to enhance recovery outcomes Educate patients and families regarding therapeutic objectives, discharge plans, and home exercise regimens Document evaluations, treatments, and patient progress in compliance with facility standards and regulations Participate actively in care team rounds and interdisciplinary team conferences to ensure coherent care coordination Education Doctorate in Physical Therapy (DPT) or an equivalent degree from a recognized institution Certifi
