Description Assess patients in both inpatient and outpatient settings and develop personalized treatment plans Implement physical therapy techniques that enhance mobility, strength, balance, and overall functionality Work alongside nurses, physicians, case managers, and other rehabilitation specialists to maximize patient outcomes Guide patients and their families in understanding therapeutic goals, discharge plans, and home exercises Maintain detailed documentation of evaluations, treatments, and progress according to facility and regulatory standards Engage in care team rounds and interdisciplinary meetings to ensure cohesive and coordinated patient care Education Doctorate in Physical Therapy (DPT) or equivalent from a recognized institution Certifications State licensure as a Physical Therapist Preferred: CPR certification
