Job Responsibilities Conduct thorough evaluations of patients in both inpatient and outpatient settings, creating tailored treatment plans for each individual Implement physical therapy interventions that enhance mobility, strength, balance, and functional abilities Work hand-in-hand with nurses, physicians, case managers, and other rehabilitation professionals to achieve optimal patient outcomes Guide patients and their families on therapeutic objectives, discharge protocols, and at-home exercise routines Maintain accurate documentation of evaluations, treatments, and patient progress adhering to all facility and regulatory requirements Engage in care team meetings and interdisciplinary conferences to promote seamless care coordination Qualifications Doctorate in Physical Therapy (DPT) or a similar
