Job Overview We are currently seeking a compassionate, outgoing, and motivated LMSW to join our growing healthcare team as an ECM (Enhanced Care Management) Social Worker. The ideal candidate must be a strong people person who is comfortable interacting with patients from diverse backgrounds and helping connect them with community resources and support services. Responsibilities Include: Coordinate care for high-risk and complex patients Conduct patient assessments and develop individualized care plans Assist patients with accessing housing assistance, food banks, shelters, transportation, and community resources Collaborate with providers, case managers, health plans, and healthcare teams Maintain accurate and timely documentation Support patients with behavioral health and other social determinants of health needs Follow ECM program guidelines and compliance standards Build positive relationships with patients and provide compassionate support Qualifications: Active California LMSW or ACSW registration required Must be personable, professional, and patient-focused Strong communication and organizational skills Knowledge of housing assistance programs, food banks, shelters, and community resources required Familiarity with IEHP and ECM programs preferred Healthcare, case management, or managed care experience preferred Ability to work independently and as part of a team Bilingual preferred We are looking for someone who is compassionate, organized, resourceful, and dedicated to improving patient outcomes and quality of life.
