Executive Director of Clinical Programs About the Company Reputable healthcare & social services organization Industry Hospital & Health Care Type Non Profit About the Role The Company is in search of an Executive Director of Clinical Programs to lead the clinical strategy and integration of care models that address both medical needs and social determinants of health. The successful candidate will serve as the clinical leader for a multi-partner care network, driving collaboration between medical providers, health plans, and community-based organizations. This executive-level role requires the design and advancement of clinical frameworks, policies, and operational standards to support effective partnerships, as well as the identification and development of new programmatic opportunities focused on prevention, chronic disease management, and evidence-based interventions. The Executive Director will also be responsible for monitoring healthcare policy, payment reform, and market trends to inform the organization's strategy. Applicants must be Nurse Practitioners with an active New York State NP license, and a clinical background in primary care is strongly preferred. The role demands a demonstrated understanding of value-based payment models, healthcare financing, and experience in evaluating program outcomes, quality metrics, and return on investment. Knowledge of regional demographics, community health needs, and health disparities is essential, as is the ability to support advocacy, community engagement, or public-facing initiatives. The ideal candidate will have the ability to collaborate effectively with executive leadership and cross-functional teams, and to guide innovation at the intersection of primary care and social services with a focus on improving outcomes and reducing disparities.
